Thank you for creating a new account. Before Continuing, it is important that you understand what type of account that you will be creating and what information is required to complete your new account.

Dream Referral has two different account types:


HME Supplier Account

Choose this type if you are an HME Supplier, Pharmacy, Independent Diagnostic Testing Facility, Sleep Lab, or any other type of company that will be receiving referrals from a physician.

Required Information – In order to complete the setup for an HME Supplier Account the following information is required. Please be sure that you have this before proceeding. Supplier Name, Address, Contact Person, Phone, Fax, Supplier NPI Number, Insurance Providers accepted and Products Offered.

Medical Practices

Choose this type of account if you are a licensed physician and will be using Dream Referral for e-prescribing of Home Medical Equipment and supplies, Physicians Nurse, Discharge Planner, Sleep Lab Technician, or other Physician Administrative Personnel. This type of account will allow the user to enter all patient information and complete a referral but will not allow the user to sign a physician order for a patient.

Required Information – In order to complete the setup for a Physician Support Account, the following information is required. Please be sure that you have this before proceeding; Physician Practice Name, or Facility Name, Contact name, address, phone, fax and NPI number.
